HEALTH BENEFITS


Medical.

Sun provides health benefits to eligible employees and their eligible dependents, which includes spouse or domestic partner and children and/or the children of a domestic partner.

Choose from several health care options:

  • Aetna Health Fund - a Consumer Driven Health Plan with a Health Reimbursement Account. This is a Point of Service plan which offers in-network and out-of-network coverage.

  • UnitedHealthcare Choice Plus - This is a preferred provider organization to employees who live outside of California, which offers in-network and out-of-network coverage.

  • Blue Cross PPO - This is a preferred provider organization available to employees in California, which offers in-network and out-of-network coverage.

  • Out-of-Area Option - This option is offered in areas where the UnitedHealthcare Choice Plus or Blue Cross PPO is not available.

  • HMO options - Kaiser Permanente in CA and CO, and Tufts Health Plan in MA.


Aetna Health Fund: The plan option is comprised of three components:
  • Health Reimbursement Account (HRA) - The HRA is an account for each enrolled employee to use toward eligible medical expenses that are covered by the plan, including the calendar year deductible. Sun funds this account with a set dollar amount each year. When you or an eligible dependent incur a covered medical or prescription drug expense and you haven't met the calendar year deductible yet, your HRA pays the full amount of the covered expense up to the amount available in your account.

  • Deductible - The calendar year deductible is the amount you must pay before the medical plan coverage pays any benefits. When all of your HRA funds have been used to pay for covered expenses, you are then responsible for the balance of your deductible that was not covered by the HRA. Preventive care services as well as prescription drugs that Aetna considers "Chronic" and "Preventive," do not reduce the HRA or apply to the deductible. These expenses are automatically covered under the medical plan.

  • Medical Plan - Once you use up the money in your HRA and you have met the deductible, eligible expenses are covered under the medical plan. You are responsible for any applicable coinsurance and out-of-pocket expenses not covered by the plan. Because the plan promotes wellness, preventive care services have no calendar year maximum and are not subject to the deductible.

INSURANCE

  • Short-Term Disability Program - replaces 75% of your pre-disability compensation for up to 90 days every rolling-back 12 month period for one or more disabilities. A rolling-back period is the 12 months immediately preceding the start of the current disability.

  • Long-Term Disability - replaces 50% or 65% of your pre-disability compensation (depending on your Sun Flex enrollment election), up to a maximum benefit of $15,000 per month if you are disabled due to illness or injury. Begins after 90 consecutive days of total disability.

Life Insurance - designed to pay a benefit to your beneficiary if you die while you are employed at Sun.

  • provides you with a basic coverage of $50,000 and

  • a choice to add supplemental life insurance in an amount of one, two, three, four, or five times your eligible compensation

  • a choice to cover your spouse/domestic partner and children

  • provisions for accelerated payout of benefits, tuition benefit for your surviving spouse/domestic partner and children, and a childcare center benefit and additional payout for use of a seatbelt and/or airbag.

Accidental Death and Dismemberment - designed to pay a benefit if you should die or suffer certain injuries due to an accident. The plan offers:

  • a choice from several AD&D options ranging from $50,000 to $500,000

  • a choice to decline coverage

  • a choice to cover your spouse/domestic partner and children

  • provisions for additional payouts for claims resulting from coma, natural disaster benefit, or claims incurred while using a seat belt and/or airbag benefit.


Business Travel Accident - provides up to $400,000 for accidental death and dismemberment from covered accidents occurring anytime you are traveling on Sun-approved business.

FINANCIAL


401(k) Plan.

  • Before -Tax Contributions: You can contribute from 1% to 30% of your before-tax pay up to a maximum of $15,500 for 2008.

  • Matching Contributions: Sun matches your contributions dollar for dollar on the first 3% of pay you contribute, plus 50 cents on the dollar for the next 2% of your pay up to an annual maximum of $6,800.

  • Catch-up Contributions: You are eligible to make catch-up contributions if:

    • You are age 50 or older, or will be by December 31;

    • You contribute the maximum 401(k) dollar limit of $15,500 or 30% of eligible compensation during the plan year; and

    • You may defer up to 30% of your eligible compensation (in addition to regular before-tax contributions) up to the annual dollar limit of $5,000

The maximum contribution between your regular 401(k) and Catch-up contributions can't exceed $20,500 for 2008. Your contributions are deducted from your pay before federal (and state, in most locations) income tax withholding is calculated.


Investment Fund Choices - Choose to invest your contributions among several funds:
  • Tier One - Target Retirement Funds offers 11 fund choices

  • Tier Two - Offers 14 investment options with different mutual funds.

Loans. You can take a loan of up to 50% of your account balance, up to a maximum of $50,000


Employee Stock Purchase Plan

  • Contribute up to 10% of your pay to purchase stock

  • Purchase price is 85% of fair market value on the stock purchase date

Flexible Spending Accounts. There are two Sun Flex Spending Accounts that let you pay for certain health and/or dependent care expenses on a before tax basis:

  • Health Care Spending Account for eligible medical, dental, prescription drug, vision and mental health care expenses incurred by you or your eligible dependents: contribute up to $5,000 to your Health Care Spending Account per calendar year

  • Dependent Care Spending Account, called SunCare, for eligible dependent day care expenses contribute up to $5,000 to your SunCare (dependent care) account per calendar year

EMPLOYEE SERVICES

  • Adoption Assistance - helps you offset the cost of adopting a child. Sun pays up to $2,200 (gross of FICA taxes) for each adoption. This reimbursement is not generally subject to federal or state withholding. You are eligible for this benefit upon your first day of employment.

  • Employee Assistance Program (EAP) - confidential counseling and referral for you and your family. You can visit an EAP counselor up to eight times each year at no charge. EAP services are only offered through the network of EAP providers available.

  • SunBalance - Sun's work and family consultation/referral program which provides practical advice on child care, adoption, elder care, education, working and parenting issues and seminars dealing with personal and business issues.

  • FIT@SUN - fitness centers and wellness programs which offer fitness training, nutritional guidance, stress management strategies and a series of informational lunch programs that focus on health, wellness, fitness and medical topics.

  • Lactation Program - provides lactation seminars and lactation rooms at various Sun locations.

  • Employee Equipment Purchase Plan - Sun employees can buy select company products at a discount.

  • Service Recognition Awards - recognition and service awards for employees as they reach five year service milestones (5, 10, 15, etc.)

  • Credit Union - Sun is associated with the Bellco First Federal Credit Union, the Technology Credit Union and the USAlliance Federal Credit Union.

  • Child Care - Sun has partnered with Bright Horizons and KinderCare to provide employees with primary, back-up and school-age child care options.

  • Fun@Sun - provides discounts for Sun employees for a variety of services, recreation and entertainment attractions.

  • Matched Giving Program - gives every Sun employee the opportunity to have donations to their favorite nonprofit organizations doubled.

  • Career Services - provides web-based resources to help you identify and utilize your unique talents and abilities, expand your capabilities, and make your work experience enjoyable and rewarding.

  • Tuition Reimbursement - provides academic advising and pays up to $5,250 per calendar year toward your expenses for and approved course of study.

  • Employee Referral - pays you a cash bonus when you refer a new employee. The bonus is payable after the new hire completes 30 days of employment.
